Clinical Manifestation of Major Arterial Injuries of The Extremities

Abstract
Major arterial injuries of the extremities are very serious injuries for limb salvages and are cause bad complications of invasive studies. The purpose of this study is to establish a plan for optimally managing major arterial injuries of the extremities. The author analyzed the records of 43 cases of reconstructive vascular surgery for major arterial injuries of upper and lower extremities at the Gil Medical Center from September 1994 to August 1998. The clinical presentations were as follows; absent or decreased pulsation in 24, cool ischemic extremity in 32, large expanding hematoma in 9, and major bleeding in 4 cases. The causes of injuries included stab wounds in 15, traffic accidents in 9, industrial accidents in 8, fallings in 2, and complications of invasive studies in 9 patients. The types of arterial injury included transections in 22, lacerations in 9, contusions with subsequent thrombus in 2, and pseudoaneurysms in 10 cases. Arterial injuries in combination with muscle injuries were present in 31, with bone fracture in 15, with nerve injuries in 12, and with vein injuries in 19 cases. The arterial reconstructions, including 23 cases of vein interposition graft and 4 end to end anastomosis were performed. The factors for successful management of major arterial injuries of the extremities are early diagnosis, prompt surgeries, and simultaneous treatment of concomitant injuries.
